If this is your first time applying for a US passport, you will need to appear in person at your local passport acceptance facility.  You will also need to appear in person if: your previous US passport was lost or damaged; your previous US passport is expired and was issued more than 15 years ago; your previous US passport is expired and was issued before you turned 16; you changed your name and you do not have a legal document demonstrating this change.&lt;/p&gt;
